Mission Statement
It is our mission at Extended Family Home Care to provide paraprofessional services to patients in their place of residence to ensure their highest level of independence while maintaining their safety. We are here to value, respect, and commit ourselves to improving the dignity and quality of life of every one of our clients.

Our Executive Team
Caleb Horne
CEO, Managing Partner
Caleb has a Bachelor’s degree in Public Safety Management from Florida Gulf Coast University and holds the following active licenses in the state of Florida.

Caleb has extensive pre-hospital medical experience, serving 11 years as a fire department lead medic and 12 years as a paramedic on critical care flights. He also spent three years as a Paramedic Field Training Officer in medical transport. Passionate about health and safety, he is dedicated to meeting community in-home healthcare needs.
Syomara Horne
CFO, Managing Partner
Syomara has a Bachelor’s degree in Business Administration with a major in Accounting and a minor in Management Information Systems from the University of South Florida. Syomara has over 20 years of experience in finance, accounting, and operations. After earning her degree in 2002, she worked as a staff accountant in both private and government sectors. She later founded and led an IT hardware company for five years. Over the past nine years, she has held executive roles, including Controller, VP of Operations, and CFO, while also consulting on project management, operations, and business efficiency.
Syomara is true born leader. She has a passion for training and developing her team members, motivating them to reach their highest potential. She is dedicated to ensuring that the administrative and compliance standards of the home health care industry are not only met but exceeded.

Jackie Tran
Controller
Jackie was raised and has spent most of her life in Tampa, Florida. She holds a Bachelor’s degree in Accounting from the University of South Florida, which she earned in 2018. Since then, she has built a well-rounded career working in finance, accounting, and human resources within private companies. Her experience spans both the staffing and manufacturing industries, where she has supported a wide range of administrative and compliance functions. Recognized for her approachable nature, attention to detail, and drive, Jackie is committed to fostering organized and effective workplaces. Outside of work, she loves to travel, read, and challenge herself with puzzles—all of which reflect her curious nature and problem-solving mindset.

Kelly Barnes, RN
Nurse Supervisor
Kelly has an Associate’s Degree of Nursing from Excelsior College in Albany, NY. Kelly became an LPN in 1994 and later became an RN in 2004 when she completed her Associate’s. She has 30 years of nursing experience, with 8 of those years specifically in the home health sector. Kelly takes pride in motivating her team to be the best in patient care & satisfaction, cultivating talent, and encouraging development. She has a passion for the advocacy of her team and her patients, while implementing critical thinking and clinical judgement skills. She strives to make the best home health experience for clients and their families.

Michelle Pittman, RN
Nurse Supervisor
Michelle, is a dedicated healthcare professional with over 20 years of experience in nursing, esthetics, and massage therapy. As a Registered Nurse with a strong background in home health care, she brings expertise in patient assessments, care coordination, and staff training. Michelle has worked across various settings, including private duty home care, wellness programs, and healthcare clinics. Her compassionate approach to enhancing the quality of life for clients align perfectly with the values of Extended Family Home Care. Michelle is passionate about providing personalized care and supporting families in navigating their healthcare needs.
Lee Sanchez
Senior Care Coordinator
Lee is a compassionate and dedicated Patient Care Coordinator demonstrating over 7 years of providing optimal care as a Home Health Aide and Office Coordinator. She is committed to providing all patients with outstanding care and scheduling aide support to help improve their quality of life. She is friendly and genuinely desires to help others in all needs to help improve their home health experience and enjoy enriching their life at home.

Dana Herrera
Home Care Liaison
Dana Herrera is a dedicated and versatile home health care professional with over 8 years of experience excelling as an HR Director, Marketer, Alternate Administrator, and Physician Liaison. Dana is passionate about helping people feel supported, valued, and at ease. She is driven by a genuine love for connecting with others and believes that compassion, trust, and communication are at the heart of great care. Known for her warmth and positive energy, Dana takes pride in building meaningful relationships with families and the community. Family is at the center of her life, and that perspective shapes the care and dedication she brings to her work every day.

Lindsey Linck
Home Care Liaison
Originally from Kansas, Lindsey is a proud Kansas State University graduate (EMAW!!), where she earned her Bachelor’s degree in Elementary Education. She began her career in North Carolina, spending 15 years inspiring young minds as a 7th-grade science teacher. After relocating to Florida, Lindsey embraced a new professional chapter in the hearing aid industry, serving first as a Patient Care Coordinator and later as Office Manager for Hear Again America. It was there that she discovered her true passion: helping seniors live full, meaningful, and independent lives. Motivated by this calling, Lindsey began volunteering with Senior Friendship Centers, where she gained a profound understanding of the needs of seniors in the community—solidifying her decision to pursue a career dedicated to serving them. With the patience of a seasoned middle school teacher and a natural ability to connect and communicate with older adults, Lindsey brings compassion, organization, and heart to her role as Home Care Liaison making her a seamless and impactful addition to the Extended Family Home Care team. Outside of work, Lindsey is an active volunteer with organizations including Senior Friendship Centers, All Faiths Food Bank, St. Barbara’s Greek Festival, Sertoma, and Girls Inc. She loves spending time at the beach, adores animals, and—above all—cherishes spending time with loved ones.
